Not Getting Proper Print Format

    Hello Experts,
    I am not getting proper print format for 2nd page as attached screen in Address Book Report
    And i want to print Header with 1st Line Company Name & 2nd Line Report Name, No Date range or any other.
    Please guide thanks

    [Part: My Cl PGBR] ;; Closing Page Break
